DVD Purchase Procedure
Purchaser may obtain a request form (FOIA form) in the Communications Department. The Communications Department is located in the Warren Community Center at 5460 Arden, Warren, Michigan. A staff member will calculate and fill in the total fee.
Purchaser must then bring the completed and signed FOIA form to the Parks & Recreation Department’s main desk at the Community Center and make payment.
Once the requested copy is ready for pickup, a staff member will contact the purchaser.
